The first visit is all about you! Dr. Nicole will complete a thorough health history, examination, and posture analysis. This allows her to learn more about you, your health, and your goals so she can determine how Rise Chiropractic & Nutrition can help you. Dr. Nicole will review all of your findings, and together you will create a plan to help you achieve your goals. You will receive treatment at the end of this visit as long as no other testing is needed.

It is important for Dr. Nicole and Jay to have your completed paperwork 24 hours prior to your visit so that they can review your answers to be prepared for your visit. Rise Chiropractic & Nutrition is a self-pay office. This means that we do not directly bill your medical insurance and that you are responsible for payment at the time of service. As a courtesy to you, we are happy to provide you with a detailed receipt that you can submit to your insurance to request reimbursement.

We accept Health Savings Accounts (HSA), Flexible Spending Accounts (FSA), check, and credit/debit cards as forms of payment.

Choosing to be self-pay allows us to create plans that are customized to your goals and needs. It also allows us to spend longer amounts of time with each patient, and the ability to include many complimentary services with our adjustments.

Please note: at this time, we are non-participating with Medicare, but at this time are NOT accepting new Medicare practice members. If you need a referral please contact us.

At this time, we are not accepting new patients who have been injured at work or in an auto accident. If you are a current patient and this situation applies to you, please contact us so we can take the next best steps to get you back to health.

Your appointment time is reserved just for you. We understand things happen, but a late cancellation or missed visit leaves a hole in the day that could have been filled by another practice member who needs it. As such, we require a minimum of 12 hours notice for any cancellations or changes to your appointment. Practice members who provide less than 12 hours notice, or miss their appointment, will incur a $40 fee.

We will do our best to accommodate, but in fairness to all of our practice members, appointments will always begin as scheduled and if  you arrive late or with incomplete paperwork, you may lose a portion of your treatment time.

No problem! We offer a complementary consultation which is a 20 minute Telehealth call for us to get to know one another and have all of your questions answered. On this call we will determine what the next best steps are to help you reach you or your child’s goals. If you aren’t quite ready to book your New Practice Member appointment, start by booking you Complementary Consultation here

That must mean that we are with practice members and coaching clients. We do our best to answer the phone throughout the day, but if we are seeing practice members and clients we may not be able to get to the phone in time. Please leave a message, or send an email to contact@risechiropracticandnutrition.com. We will get back to you as soon as possible!
