COVID-19 Policies

Rise Chiropractic & Nutrition is following all guidelines and regulations set forth by the State of Ohio, the CDC, and following guidance from the Ohio State Chiropractic Association. 

In order to keep appointments simple, safe and stress-free, here is what you can expect from your visit:

    • -You will be asked a few self-screening questions when completing your intake forms prior to your appointment.

    • -As per the CDC’s most recent guidelines, masks are not required to be worn in the office; however, we will have masks available if you’d like to wear one.

    • -Please be mindful of other patients and respectful of the choice they choose to make while you’re in the office.

    • -If you are feeling unwell, have been at an exposure site, or experiencing any unusual symptoms, we ask that you reschedule your appointment. We will also reschedule appointments if the practitioner or staff is unwell.

Booking & Payment

We are open by appointment only. Same day appointments are possible.

The easiest way to book your appointment is through our online booking site. You may also schedule by calling/texting 614-681-0195, or you may e-mail contact@risechiropracticandnutrition.com.

We accept all major credit cards, HSA/ FSA cards, and check as forms of payment. We also offer contactless payment for your convenience. All payment is due at the time of service. 

We are a self-pay office, but as a courtesy to you, we will provide a detailed receipt that you can submit to your medical insurance for reimbursement. 

Cancellation Policy

Your appointment time is reserved just for you. We understand things happen… but a late cancellation or missed visit leaves a hole in the schedule that could have been filled by another practice member who needs it. As such, we require a minimum of 12 hours notice for any cancellations or changes to your appointment. Patients who provide less than 12 hours notice, or miss their appointment, you will incur a $40 charge. 

In fairness to all of our practice members, appointments will always begin as scheduled. If you arrive late or with incomplete paperwork you may lose a portion of your treatment time.

Paperwork Policy

If your appointment requires paperwork to be completed (New Practice Member and Reassessment), it is required to have your completed paperwork submitted 24 hours prior to your appointment so Dr. Nicole & Jay can best prepare for your appointment. If paperwork is incomplete, you may be asked to reschedule.
